Preservation of chordae during MV replacement

2D apical 2-chamber, LV

(tt2857) MV chordae to the posterior leaflet are spared (thick white arrow) during MV replacement with bioprosthesis (thin white arrows point to the cusps of the mitral bioprosthesis), to preserve anatomical and functional continuity between the LV longitudinal fibers of the postero-medial papillary muscle and the mitral annulus. Preservation of LV longitudinal function is essential to preserve LV global function (ejection fraction) after mitral valve (replacement or repair) surgery.
